Will you bill my insurance?

 

Yes, as a courtesy to our patients we will bill all insurance companies. In order to offer this courtesy, we will need a copy of your current insurance card. If you do not have a current insurance card, we will give you a copy of the encounter form for you to submit to your insurance and payment will be expected at time of service.

 

Do I have to pay my copay at time of service?

 

Yes. Our contract with your insurance states we must collect all copayments from the insured. If you are not able to pay at time of service, we will charge an administration fee of $5.00 to cover our costs to process and mail a statement. For your convenience we accept cash, Check, Visa, MasterCard, American Express and Discover Card.

 

Does my insurance cover this visit?

 

If you are a new patient we do verify your insurance coverage and benefits. This is not a guarantee of payment as stated by your insurance company. It is recommended that you check your benefits prior to your visit.
